Does your Dog Have Current Rabies Vaccination?

In consideration of my participation in the Pamper & Pics for Pups event, I undersigned acknowledge and agree to the following:

• I, my doggy, nor has any member(s) of my household, experienced symptoms of fever, fatigue, difficulty in breathing, persistent chest pain or dry cough or exhibited any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.
   

• I, my doggy, nor have any member(s) of my household, traveled by sea or by air, internationally within the past 30 days.
   

• I, my doggy, nor have any member(s) of my household, been diagnosed to be infected with COVID-19 virus within the last 30 days.
   

• I agree that my doggy does not have a history of aggression towards other pets or people and if aggressive behavior is displayed, he/she will not be allowed participation in the event. I understand that Wise Wags/newNsight Photography reserve the right to omit, restrict or halt service to any pet due to aggression, aggressive body language or uncontrollable wiggling, growling, snarling, barking etc.
   

• I am fully and personally responsible for my, my doggy's  and minors safety and actions while and during my participation and I hereby release, waive, discharge Wise Wags, newNsight Photography, their board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or illness, that may be sustained.

• I understand  this is a promotional event and I, my doggy and members of my group will be photographed and /or recorded during my participation. I hereby assign newNsight Photography and Wise Wags the irrevocable and unrestricted right to use and/or publish photographs/footage that may include me, my doggy and any members of  my group for editorial, trade, advertising, educational and promotional use; to alter the same without restriction; and to copyright the same without restriction.
   

• I will be prepared to reconfirm these statements along with any other adults attending with me the day of the event by signing a waiver and release form prior to participation.
